The following form includes all the information you need to register and prepare for the 2024-2025 school year. Access the checklist here!

Complete the Willard School District Health Information Form. This health form must be submitted each school year. Please note that there is no way to submit it directly online. You must download and complete the form and email/fax/mail it to your students' school nurse or drop off a hard copy.

All new and returning students in need of school transportation must complete the following forms. Even if your child has previously ridden the bus, you will still need to fill out the Request for Bus Transportation.

All Willard families are encouraged to complete the Free & Reduced Lunch Application form each school year. Even if you do not qualify, your record may benefit Willard students and its programs.

Information provided on the application is confidential and will be used only for the purpose of determining eligibility. If possible, please fill the form out online through the LINQ Connect Portal. The district will notify you within 10 school days, either by email or letter, if you qualify for free or reduced meals. If you do not receive a notice after 10 days, please contact Willard Nutrition Services Department at 417-742-2584.

Missouri Law requires children enrolled in Kindergarten through 12th grade to be immunized against certain preventable diseases. Updated immunization records are required for students entering kindergarten, 8th grade, and 12th grade. All students must present documentation of an up-to-date immunization status, including month, day, and year of each immunization before attending school.
